Underwriting Considerations

Employer Eligibility

Employers must have operations in an approved state and meet the state mandated minimum participation in that state. Each employer must complete an Employer Participation Agreement and is subject to final underwriting approval.

Offering More Than One Plan

Employers with 51 to 250 employees may elect to offer 2 plans and employers with more than 250 employees may elect to offer any 3 of the Healthy Basic Plans. Customized plans are available for larger employers.

Employer Contribution & Plan Rates

Employers may elect to pay from 0% to 100% of premium for selected plans. There are 2 plan rate tables available for each plan depending on employer contribution. There is a discount on the premiums for employer contributions of 50% or more toward the employee only premium.

Medical Underwriting & Pre-Existing Conditions

Plans are not medically underwritten and all eligible participants will be accepted for coverage if application is made when initially eligible.

A pre-existing condition clause will apply to pregnancy when conception occurs prior to the Covered Person’s effective date of coverage in many states. Please check with your agent. No other medical conditions are subject to pre-existing condition restrictions.

Prescription Drug Benefit

A prescription drug benefit can be included. You must utilize a network pharmacy. Most major retail drug outlets are included. A complete listing will be provided with the enrollment materials.

PPO Network

Healthy Basics includes a national network of medical care providers. Using the network is optional, however, by utilizing a network provider, the Healthy Basics benefit covers more of the total medical costs. Should a specific benefit be exhausted for the calendar year, the participant will still receive the benefit of using a network provider since they provider will continue to bill using the pre-negotiated discounted rates!

Monthly Billing

Employers will receive a montlhy billing for benefits under the Healthy Basics Plans.
